This form is typically used by an Attorney (Utah) or Title Agency when the original Beneficiary/Lender sends a written request for full reconveyance. This form can be used by the original trustee or successor trustee to reconvey but without any covenant or warranty.

(Any trust deed, substitution of trustee, assignment of a beneficial interest under a trust deed, notice of assignment of a beneficial interest, notice of default, trustee's deed, reconveyance of the trust property, and any instrument by which any trust deed is subordinated or waived as to priority, if acknowledged as provided by law, is entitled to be recorded.) The Beneficiary Deed is easy to fill out, expecially with the examples/explanations provided. The only recommendation I would make is to state that the Parcel ID and the Assessor's ID are one in the same. I looked everywhere for something that mentions "Assessor's ID" in my paperwork to no avail. Upon calling the Maricopa Assessor's number in Maricopa I was told that they are the same.

We aimed to handle a survivorship affidavit (deed change) without a lawyer following my dad's death. After some searching, deeds.com seemed to have the most comprehensive and "correct looking" form we could find for our locale, so we went with it, and it was accepted by our recorder's office.

I'm not sure why our local government office doesn't offer a standard form, but they don't, and deeds.com came through for us in a pinch.
