No instrument by which the title to real estate or any interest therein or lien thereon, is conveyed, created, encumbered, assigned or otherwise disposed of, shall be recorded by the county recorder or registered by the registrar of titles until the name and address of the person who or corporation which drafted the instrument is printed, typewritten, stamped or written on it in a legible manner.

Forms used, created quitclaim deed that the county accepted without a second look (turns out they see deeds.com forms regularly via erecording and in person). Quit Claim deed for North Carolina did not include all of the information I needed (two separate notary sections), but I was able to re-create another notary section in Word, and then insert it in the appropriate place after printing both documents. Otherwise, the document worked as described.
